The world is closely watching an ongoing outbreak of Bundibugyo virus disease, a severe form of Ebola, predominantly affecting the Democratic Republic of Congo (DRC). While the DRC faces persistent challenges, its neighbor Uganda has made significant strides towards containing the virus within its borders. This complex health emergency demands a coordinated international response, bringing together health organizations and communities to stem the spread and save lives. ## Understanding Bundibugyo Ebola Bundibugyo virus disease (BVD) is a serious illness caused by one of the species within the *Orthoebolavirus* family, commonly known as Ebola. It's a zoonotic disease, meaning it originates in animals and can spread to humans. Scientists suspect fruit bats are the natural carriers of the virus. Human infection typically occurs when individuals come into close contact with infected wildlife, such as bats or certain primates, or their bodily fluids. Once in humans, the virus spreads easily from person to person through direct contact with blood, secretions, organs, or other bodily fluids of an infected person, or contaminated surfaces and materials. Healthcare settings can become hotbeds for transmission if infection prevention and control (IPC) measures are not rigorously followed. Additionally, traditional burial practices that involve direct contact with the deceased can accelerate the virus's spread within communities. Symptoms of BVD can be deceptive in their early stages, often resembling common illnesses like malaria. The incubation period, or the time between exposure and symptom onset, can range from two to 21 days. Infected individuals are not contagious until they begin to show symptoms, which initially include fever, tiredness, muscle aches, headaches, and a sore throat. As the disease progresses, these symptoms can worsen, leading to severe gastrointestinal issues, organ failure, and in some cases, internal and external bleeding. Without prompt medical care, the disease can be fatal. Past outbreaks in Uganda and the DRC saw fatality rates of 30% and 50% respectively, underscoring the severity of this particular Ebola strain. ## The Current Outbreak in Democratic Republic of Congo As of July 15, 2026, the Democratic Republic of Congo continues to battle an active Bundibugyo virus outbreak. Health authorities have reported a cumulative total of 2,124 confirmed cases, tragically including 828 deaths. This translates to a case fatality ratio of 39%, highlighting the deadly nature of the virus. The outbreak remains active across 38 of the 46 affected health zones, meaning these areas have reported new cases within the last 21 days. The province of Ituri is particularly hard-hit, accounting for nearly 90% of all confirmed cases and over 83% of all reported deaths nationwide. Response efforts face immense challenges in the DRC, a country grappling with complex humanitarian issues and ongoing conflict. High population mobility, including displaced communities with limited access to essential services like clean water, food, and healthcare, amplifies the risk of transmission. Security incidents often disrupt critical surveillance and response activities, making it difficult for health teams to reach affected populations and increasing the chances of undetected spread. The scale-up of surveillance, testing, and diagnostic capabilities has contributed to the increase in reported cases, indicating improved detection rather than necessarily a sudden surge in new infections. Concerningly, infections among healthcare workers continue to rise, with 119 confirmed cases and 36 deaths reported, pointing to persistent occupational exposure risks and the critical need for enhanced infection prevention and control in health facilities. ## Uganda's Progress Towards Containment In contrast to the DRC, Uganda has demonstrated remarkable progress in controlling the spread of the Bundibugyo virus. The country has not reported any new confirmed cases since June 21, 2026. A total of 20 confirmed cases were identified, with two deaths occurring in imported cases. All cases in Uganda were reported in the Kampala District, and importantly, no community transmission has been detected. The cases identified were either imported from the DRC or secondary cases linked to these imported individuals, often involving healthcare settings or cross-border movements. Following two negative test results, the most recent patient was discharged from a treatment center on July 16, initiating Uganda's crucial 42-day countdown period. This period, which is twice the maximum incubation period of the virus, allows for intensive surveillance to ensure no new cases emerge before the outbreak can officially be declared over. Despite this positive development, the ongoing outbreak in the neighboring DRC means Uganda remains at a high risk for new imported cases, necessitating continued vigilance and robust border health measures. ## International Cases and Global Vigilance The cross-border nature of the outbreak has led to several international cases. A humanitarian worker from the United States, who contracted the Bundibugyo virus in the DRC, was medically evacuated to Germany in July 2026 for treatment and is reported to be in stable condition. This marks the second instance of a US citizen being treated in Germany for the virus, with a physician having recovered earlier in May 2026. France also reported an imported confirmed case on June 24, with the patient recovering and being discharged on July 4. Rigorous contact tracing and monitoring of individuals who had low-risk exposure to the French case confirmed no secondary transmission. These international cases underscore the importance of rapid global collaboration in managing highly infectious diseases and ensuring specialized medical care for those affected. ## How Bundibugyo Virus Spreads and Its Symptoms The Bundibugyo virus is primarily transmitted through direct contact with the bodily fluids of an infected person or contaminated objects. This can occur through broken skin or mucous membranes in the eyes, nose, or mouth. Understanding the transmission routes is critical for prevention. Unsafe healthcare practices, such as reusing needles or insufficient sterilization of medical equipment, can amplify spread. Traditional funeral rites that involve close physical contact with the deceased are also significant risk factors. Early symptoms are often non-specific, making clinical diagnosis challenging without laboratory confirmation. This delay in identification can allow the virus to spread further before appropriate isolation measures are put in place. Laboratory tests, such as PCR or antigen/antibody-based assays, are essential for accurate diagnosis and differentiating BVD from other febrile illnesses like malaria. Currently, there are no approved vaccines or specific treatments for Bundibugyo virus disease, making rapid case identification, isolation, supportive care, diligent contact tracing, and safe burial practices the cornerstones of outbreak control. ## Protecting Communities: The Public Health Response In response to the Bundibugyo virus threat, health authorities in both the Democratic Republic of Congo and Uganda, in partnership with the World Health Organization (WHO) and other international collaborators, are implementing extensive public health measures. These efforts are guided by a continental response plan and involve mobilizing resources to address critical funding gaps. Key activities include strengthening surveillance systems to detect new cases quickly, ensuring rapid laboratory testing, providing compassionate and effective care for patients, and meticulous contact tracing to identify and monitor individuals who may have been exposed. Infection prevention and control (IPC) protocols are being reinforced in healthcare facilities to protect patients and health workers alike. Community engagement is paramount, ensuring that local leaders and populations are actively involved in understanding the virus, adopting preventive behaviors, and supporting response teams. This community-led approach is crucial, especially in areas affected by instability, to build trust and ensure the effectiveness of public health interventions. Ongoing research into potential vaccines and therapeutics for BVD is also underway, with expert groups convened by WHO to assess promising candidates. ## Assessing the Risk and WHO Recommendations The World Health Organization consistently assesses the risk associated with the Bundibugyo virus outbreak to guide global preparedness and response. The risk in the Democratic Republic of Congo remains categorized as very high due to sustained transmission and the expansion of the outbreak into new geographical areas, which increases the potential for wider national and regional spread. Uganda's risk level is assessed as high because of confirmed cross-border spread and historical epidemiological links with the eastern DRC. Countries sharing land borders with the DRC and Uganda are also considered at high risk due to significant population movement for trade and mining, varying capacities for BVD response, and differing levels of preparedness. For the rest of the African region and at the global level, the risk is currently assessed as low. Based on current information, WHO advises against any restrictions on international travel to, or trade with, the Democratic Republic of Congo or Uganda. Instead, the emphasis is on strengthening coordinated outbreak control, enhancing cross-border collaboration, and maintaining robust surveillance and preparedness. Rapid detection of cases, accurate testing, and optimized supportive care are vital not only to reduce mortality but also to foster community trust and acceptance of essential health services. Ongoing dialogue among experts is exploring candidate vaccines and treatments, underscoring the long-term commitment to combating this challenging disease.
