Abstract
To provide an evidence-based, practical framework for the detection and management of frailty in community-dwelling older adults in primary care. Active opportunistic case finding in individuals aged ≥ 70 years using simple performance-based tools enables early identification of frailty. Multicomponent interventions integrating physical exercise, nutritional support, medication optimisation and psychosocial strategies are effective in preventing or reversing frailty.
